Volunteer Group Requirements
All volunteers must review and complete:
After you have submitted the electronic Application and Medical forms, please return a physical copy of the signed Liability Waiver to your group leader.
Group Leaders: After collecting a physical copy of the signed Liability Waiver from each participant, please mail them to the Amizade office at PO Box 6894, 343 Stansbury Hall, Morgantown, WV 26506.
