• Amizade Liability Form (pdf)
• Amizade’s Pre-Departure Survey*

After returning, all volunteers must review and complete:

• Amizade’s Online Post-Evaluation*

*Please note: you will need to have the most up to date version of your Internet browser to access these forms, we recommend Google Chrome.

After you have submitted the electronic Medical form, please return a physical copy of the signed Liability Waiver to the Amizade office at 4 Smithfield Street, Floor 7, Pittsburgh, PA 15222 or Fax it to 757.257.8358.
