• Amizade Liability Form (pdf)
• Amizade’s Pre-Departure Survey

Individual volunteers only:

• Background Check Notification and Authorization Waiver (PDF)

After returning, all volunteers must review and complete:

• Amizade’s Online Post-Evaluation

After you have submitted the electronic Medical form, please return a physical copy of the signed Liability Waiver and Background Check Waiver to the Amizade office at 4 Smithfield Street, Floor 7, Pittsburgh, PA 15222 or Fax to 757-257-8358.
